Let’s Talk About Palliative Care

Caring for a loved one is a profound act of love. While it can be deeply rewarding, it can also be emotionally and physically challenging. That’s why we invite you to start a conversation with us—to see how we might be able to help.

Even if your loved one’s goal is to remain at home for as long as possible or to die at home, an early referral to Carpenter Hospice can offer meaningful support.

A referral does not mean a move to hospice is immediate or inevitable. Instead, it opens the door to learning about our services, building relationships with our care team, and planning ahead.

Once a referral is made, our Intake Coordinator will connect with you to learn more about your needs and goals. She will collaborate closely with your healthcare providers to determine the most appropriate care options. Admissions are prioritized based on individual needs, and Laurie will support and guide you through the process with care and compassion.

How to Request a Referral

Referrals can be made by a home care coordinator, physician, hospital staff, or other healthcare professionals. Families and caregivers are also encouraged to speak with a healthcare provider to begin the referral process.

Admission to Carpenter Hospice 

Admission may be considered when a life-limiting illness has progressed to the point where curative treatments are no longer effective, and the focus shifts to comfort and quality of life.

Hospice care supports individuals in the final stages of serious illness by providing expert symptom management, along with emotional, spiritual, and practical support—for both the individual and their loved ones.

Eligibility Criteria

To be considered for admission, the individual must:

  • be 18 years of age or older
  • have a valid Ontario Health Card
  • be a resident of Burlington or have close family living in the surrounding area
